Dance Attire -
Ballet & Combo Classes - Black leotard, pink tights,
pink ballet shoes & black tap shoes.

Placement -
Students will be placed based on age, technical ability and individual
potential. Most classes will be limited to a maximum of 12 students.

Liability Release -
I understand and recognize the risk of physical injury inherent in dance
and dance performances and I am willing to assume those risks. I agree
that I will not hold Studio 3 Dance or it's directors or employees liable
for injuries sustained while in attendance and/or participating in any
class activity at Studio 3 Dance.

Payment Policy -
I understand that monthly tuition is due on the 1st of every month.
A $15 late fee will be charged after the 10th and statements are only
mailed to past due accounts. Written notification must be given before
the 1st of a month if you find it necessary to drop a dance class.
Otherwise tuition for that month will be due.
